This form is to be used for placing reservations on the Disc Golf Course for special events or large groups of participants. Reservations close the course to the public.

The undersigned will be responsible for seeing that all park rules and regulations are adhered to by the group. The group will be expected to leave the Disk Golf Course in a clean and orderly state. The group releases Boardman Park & Recreation District from all liability in all actions for which the group is responsible. Notice must be given to Boardman Park & Recreation District in the case of cancellation. Persons and groups who do no adhere to this reservation policy will not be allowed to reserve the Disk Golf Course in the future. (required)

ABSOLUTELY NO GLASS IS ALLOWED IN THE PARKS OR ON THE DISK GOLF COURSE AT ANY TIME, NO BANDS, DJ'S, OR LOUD MUSIC IS PERMITTED AT ANY TIME.
